Nurse Navigator, Home Care Health Services
Responsibilities:
- Lead patient onboarding into the Home Health Program, including patient identification, home suitability assessments, care planning, patient and family education, and coordination of safe transitions from hospital to home.
- Provide clinical leadership and oversight for home health nursing services, ensuring delivery of safe, evidence-based, patient-centered care.
- Conduct supervisory home visits, support staff development, monitor clinical competencies, and coordinate ongoing training and professional education initiatives.
- Coordinate multidisciplinary care, facilitate communication between patients, families, caregivers, and healthcare providers, and ensure an exceptional patient experience throughout the continuum of care.
- Manage staffing and service coverage to ensure timely, uninterrupted care delivery while maintaining workforce compliance, licensure, and mandatory certifications.
- Develop and maintain referral relationships with hospitals, physicians, and other stakeholders to support program growth and patient enrolment targets.
- Lead quality improvement, patient safety, risk management, and clinical audit activities, ensuring compliance with organizational and regulatory standards.
- Monitor clinical outcomes, workforce performance, and operational metrics, and prepare reports to support continuous service improvement and program effectiveness.
Requirements:
- Registered Nurse from a recognized school of nursing or University
- Current licensure with the Nursing Council of Kenya
- Certification in Basic Life Support (BLS)
- Minimum of 5 years' clinical nursing experience in an acute care hospital setting.
- Experience in Home Health Services will be an added advantage.
- Demonstrated leadership and care coordination skills
- Strong critical thinking and analytical abilities.
- Ability to work effectively within multidisciplinary teams.
- Strong organizational and time-management skills.
- Ability to prioritize multiple responsibilities in a dynamic environment.
- Self-driven with the ability to work independently.
