Clinical Pharmacist

JOB PURPOSE

  • The Clinical Pharmacist is responsible for providing clinical pharmacy services, medication therapy management, and pharmaceutical care to ensure safe, effective, and rational use of medicines within the hospital.
  • The role supports multidisciplinary patient care by reviewing prescriptions, monitoring drug therapy, promoting antimicrobial stewardship, and ensuring compliance with pharmaceutical standards and regulatory requirements. The Clinical Pharmacist also contributes to patient safety, medication error prevention, and quality improvement initiatives in line with the hospital’s patient care objectives and the Pharmacy and Poisons Board of Kenya guidelines.

 MAIN DUTIES AND RESPONSIBILITIES

Clinical Pharmacy Practice & Patient Care

  • Conduct clinical review of prescriptions and treatment sheets to ensure appropriate drug selection, dosage, interactions, contraindications, and therapeutic effectiveness.
  • Participate in multidisciplinary ward rounds to provide pharmaceutical input in patient management and treatment planning.
  • Monitor drug therapy outcomes for inpatients and outpatients, identifying and resolving medication-related problems in collaboration with the healthcare team.
  • Undertake medication reconciliation during patient admission, transfer, and discharge to ensure continuity and accuracy of medication therapy.
  • Provide medication counselling and adherence monitoring for patients, particularly those on chronic therapies.

Medication Safety & Quality Assurance

  • Conduct drug utilization reviews to promote rational use of medicines and optimize therapeutic outcomes.
  • Identify, report, and analyze medication errors, adverse drug reactions, and other medication safety indicators.
  • Participate in implementation of patient safety and medication safety programs within the hospital.
  • Ensure compliance with clinical protocols, pharmaceutical regulations, and standard operating procedures.

Antimicrobial Stewardship & Clinical Governance

  • Actively participate in Antibiotic Stewardship Programs, ensuring compliance with antimicrobial guidelines and appropriate antibiotic use.
  • Serve as an active member and provide professional advice to committees such as:
  • Medicines and Therapeutics Committee
  • Antibiotic Stewardship Committee
  • Infection Prevention and Control Committee
  • Other clinical governance or advisory committees
  • Monitor and enforce institutional antibiotic stewardship restrictions and policies.

Pharmaceutical Operations & Dispensing

  • Ensure accurate and timely dispensing of medications for inpatients and outpatients in accordance with prescriptions and established protocols.
  • Supervise and participate in sterile compounding, chemotherapy reconstitution, and extemporaneous preparations in accordance with safety standards.
  • Maintain proper handling, storage, and management of pharmaceutical products in compliance with regulatory requirements.

Education, Training & Professional Development

  • Provide drug information and therapeutic education to clinicians, nurses, and other healthcare professionals.
  • Conduct continuous professional development sessions for pharmacy staff and healthcare teams on safe medication practices.
  • Educate patients on medication use, side effects, drug interactions, and adherence strategies.

Documentation, Reporting & Compliance

  • Prepare and submit monthly, quarterly, and annual pharmaceutical reports in line with hospital requirements.
  • Maintain accurate pharmaceutical records, drug utilization reports, and clinical intervention documentation.
  • Support hospital accreditation processes, regulatory audits, and pharmacy inspections.

Collaboration & Clinical Support

  • Collaborate closely with physicians, nurses, laboratory teams, and other healthcare professionals to optimize patient outcomes.
  • Participate in hospital quality improvement initiatives related to medication management and patient safety.
  • Support implementation of clinical protocols and medication management systems across departments.

 MINIMUM REQUIREMENTS / QUALIFICATIONS

  • Bachelor’s Degree in Pharmacy from a recognized institution.
  • Valid practicing license and registration with the Pharmacy and Poisons Board of Kenya.
  • Possession of a current practicing license available for use by the hospital where applicable.
  • Minimum two (2) years’ clinical pharmacy experience in a busy hospital setting.
  • Demonstrated experience in clinical pharmacy practice, medication therapy management, and patient care support.
  • MBA or postgraduate qualification in Clinical Pharmacy or Healthcare Management will be an added advantage.

KEY JOB REQUIREMENTS

  • Strong technical troubleshooting and equipment repair skills.
  • Knowledge of medical device technology and hospital biomedical systems.
  • Ability to interpret equipment manuals, circuit diagrams, and technical documentation.
  • Strong analytical and problem-solving ability in high-pressure clinical environments.
  • Good documentation and record-keeping skills.
  • Effective communication skills to support training and collaboration with clinical staff.
  • Ability to manage multiple equipment issues and prioritize repairs effectively.
  • High level of professionalism, integrity, and accountability.
